Caregiver's Support Services Guide
Town and
Country Support Services
Transportation
Volunteer drivers provide
transportation to and from medical
appointments, personal errands, and
social activities. Fully accessible
Van services offer wheelchair
accessible transportation.
Household Help
Providing assistance with
housekeeping, shopping, and meal
preparation, and assisting with
other household tasks.
Home Maintenance and Repair
Heavy chores and home maintenance
chores (ex. yard maintenance, snow
shoveling, washing windows, etc.)
Meals on Wheels
Hot, nutritious meals delivered to
your door by volunteers up to 5
days/week. Special dietary needs can
be met.
Frozen Meals
Ready-to-heat, full meals (choice of
entrees, soups, and desserts).
Special diets can be accommodated,
with meals delivered every 2 weeks
throughout Huron County.
Seniors Dining Club
Service is provided within different
communities. The Dining Club is an
opportunity to join friends for a
hot, home-cooked meal, visiting,
entertainment and social activities.
Volunteer transportation is
available for those who need a ride.
Seniors Exercise Programs
Seniors exercise programs are
offered at several locations in
Huron, along with foot care clinics
and health and wellness sessions.
L.I.F.E. – The Living Independently
through Fitness and Exercise
programs at the Betty Cardno Center
in Clinton offers a seniors fitness
center, indoor walking club, and
other “senior friendly” programs for
active living.
Friendly Visiting
Time for conversation, activities,
and good companionship. This service
is designed to provide seniors with
regular social contact by a personal
home visit from a caring volunteer.
Telephone Reassurance
A caring volunteer will make a phone
call to people living alone to
ensure that all is well. Telephone
calls are
established at a mutually agreed
upon time, daily or otherwise.

Services
These services are offered on an as
needed basis in conjunction with
Community Care Access Centre, or can
be purchased on a private basis.

Saint Elizabeth Health Care
Support & Resources for
Caregivers
The Caregiver Support Program
provides caregivers with the skills
and support necessary to more
effectively
manage the home care needs of their
loved ones. This includes:

Caregiver relief (respite care)
free of charge for those who
need it most.

Counseling and support for the
caregiver, care recipient and/or
other family members.

Health care education that
emphasizes health and stress
management, prevention, holistic
care and care giving
instruction.
Midwestern
Adult Day Services
Adult Day programs are available in
various communities throughout Huron
County, including Clinton, Goderich,
Wingham, Exeter, and Grand Bend at a
cost of $18/day.

Seniors Transit
Transportation around the community
of Goderich with stops at various
locations on selected days. Cost is
$4 for all-day unlimited access.
Riders must be able to physically
get in and out of the vehicle on
their own, as the vehicle is not
wheelchair accessible. The driver
only stops at designated stops.
Riders must pre-register and obtain
an access card.

Cancer Society Volunteer Driving
Program
Sometimes simply getting treatment
can be a difficult task, especially
if you are also dealing with
physical or financial challenges, or
if your treatment centre is far from
home. If you need help getting to
your cancer-related care, the
Volunteer Driving Program can help
provide transportation.
South West
Community Care Access Centre
The South West CCAC helps people:
- recover from illness, injury,
or treatment at home.
- live independently at home with
extra support.
- apply for admission to a Long-Term
Care facility.
- connect with other
service/supports.
The South West CCAC can help you or
someone you care for who:
- has recently been in hospital and
still needs nursing or other care.
- needs specialized care, such as IV
therapy at home.
- is recovering from illness,
injury, or treatment at home.
- is facing the challenges of age or
disability and needs support to
manage at home.
Services
- Personal support (assistance with
personal care activities, such as
bathing or dressing).
- Nursing (wound care, teaching
related to medications and blood
pressure, ostomy care, IV therapy at
home)
- Physiotherapy (exercises,
activities to help you get moving
again).
- Occupational Therapy (advice to
make your home safer and easier to
manage).
- Speech- Language Therapy (help
with difficulties speaking,
understanding, and swallowing).
- Social work (practical and
emotional support during difficult
times).
- Nutritional counseling/Dietetics
(food management for complex health
needs).
- Medical Supplies and Equipment
Eligibility for Care at Home
- You must be an Ontario
resident and have a valid Ontario
Health Card.
- A South West CCAC case manager
must assess you as needing care.
- You must need care to stay at home
or return from the hospital.
- The care must be able to be
provided in a suitable and safe
environment.
- The care must help you make
progress toward your goals.
- The services you need must be
available in the area serviced by
the South West CCAC.
